                                INVESTMENT SUPERVISORY SERVICES ("ISS")
                                INDIVIDUAL PORTFOLIO MANAGEMENT FEES

    Our annual fees for Investment Supervisory Services are based upon a percentage of assets under
    management.

    The annualized fee for Investment Supervisory Services are charged as a percentage of assets under
    management, according to the following schedule:
                                  Assets Under Management               Annual Fee

                                        First $1.0 million                 1.00%

                                        Next $1.5 million                  .85%

                                   Next $2.5 million                .75%

                                   Next $5.0 million                .65%

                              Amount over $10 million            Negotiable

A minimum of $100,000 of assets under management is required for this service. This account size may
be negotiable under certain circumstances. Broadleaf Partners, LLC may group certain related client
accounts for the purposes of achieving the minimum account size and determining the annualized fee.

Limited Negotiability of Advisory Fees: Although Broadleaf Partners, LLC has established the
aforementioned fee schedule(s), we retain the discretion to negotiate alternative fees on a client-by-
client basis. Client facts, circumstances and needs are considered in determining the fee schedule. These
include the complexity of the client, assets to be placed under management, anticipated future
additional assets; related accounts; portfolio style, account composition, reports, among other factors.
The specific annual fee schedule is identified in the contract between the adviser and each client.

We may group certain related client accounts for the purposes of achieving the minimum account size
requirements and determining the annualized fee.

Discounts, not generally available to our advisory clients, may be offered to family members and friends
of associated persons of our firm.

                              INVESTMENT SUPERVISORY SERVICES ("ISS")
                                MODEL PORTFOLIO MANAGEMENT FEES

Our annual fees for Model Portfolio Management Services are based upon a percentage of assets under
management.

The annualized fee for the Broadleaf Growth Equity Portfolio (BGEP) and the Broadleaf Strategic
Investment Portfolios (STRIPS) will be charged as a percentage of assets under management, according
to the following schedule:

                                  First $1.0 million               1.00%

                                  Next $1.5 million                 .85%

                                   Next $2.5 million                .75%

                                   Next $5.0 million                .65%

                              Amount over $10 million            Negotiable

A minimum of $100,000 of assets under management is required for all three STRIPS model portfolios. A
minimum of $250,000 is required for the BGEP model portfolio. This account size may be negotiable

 under certain circumstances. Broadleaf Partners, LLC may group certain related client accounts for the
 purposes of achieving the minimum account size and determining the annualized fee.

 Limited Negotiability of Advisory Fees: Although Broadleaf Partners, LLC has established the

                 SELECTION and MONITORING of THIRD-PARTY MONEY MANAGERS FEES

 Broadleaf Partners, LLC's fee for this service does not include the independent investment adviser's fee
 for that entity's advisory/management services. The independent investment adviser's management fee
 is disclosed in the independent investment adviser's Firm Brochure or other disclosure document.

 Our annual fee for the Manager Selection Program is charged as a percentage of assets under
 management, as negotiated at the inception of the client relationship.

                          COMPREHENSIVE FINANCIAL PLANNING SERVICES FEES

Broadleaf Partners, LLC's Comprehensive Financial Planning Services are provided for a fixed fee of up to
$5,000, determined based on the complexity of the client's financial situation and the scope of services
agreed upon at the inception of the engagement. Factors considered in setting the fee include the number of
planning areas to be addressed, the complexity of the client’s circumstances, and the anticipated time
required to develop and present the plan. The specific fee is agreed upon in writing and disclosed to the client
prior to commencing any work and is payable in a single payment upon completion of the financial plan. This
fee is separate from and in addition to any investment management fees charged by our firm. In cases where
a significant change in the client's circumstances requires a substantial revision to a previously delivered plan,
additional fees may apply and will be communicated to and agreed upon by the client in advance of any
additional work being performed.

   Broadleaf Partners, LLC provides advisory services to the following types of clients:

          Individuals (other than high net worth individuals)
          High net worth individuals
          Pension and profit sharing plans(other than plan participants)
          Charitable organizations
          Corporations or other businesses not listed above

As previously disclosed in Item 5, our firm has established certain initial minimum account
requirements, based on the nature of the service(s) being provided. For a more detailed understanding
of those requirements, please review the disclosures provided in each applicable service.
