In addition to the information provided in the Advisory Business section, this section provides details regarding
Firm services along with descriptions of each service’s fees and compensation arrangements.

     A. FEE SCHEDULE

Lower fees for comparable services may be available from other sources.

PORTFOLIO MANAGEMENT FEES

The advisory fee is calculated using the value of the assets in the Account on the last business day of the prior
billing period.

These fees are generally negotiable, and the final fee schedule is attached as Exhibit II of the Investment
Advisory Contract. Clients may terminate the agreement without penalty for a full refund of CCAM’s fees
within five (5) business days of signing the Investment Advisory Contract. Thereafter, clients may terminate the
Investment Advisory Contract immediately at any time.

Fees are paid in advance on a quarterly basis. The advisory fee is calculated using the value of the assets in the
Account on the last business day of the prior billing period. Upon termination, for any unearned asset-based fees
paid in advance, the fee refunded will be equal to the balance of the fees collected in advance minus the daily
rate* times the number of days elapsed in the billing period up to and including the day of termination. (*The
daily rate is calculated by dividing the annual asset-based fee rate by 365.)

PENSION CONSULTING SERVICES FEES

These fees are generally negotiable, and the final fee schedule is attached as Exhibit II of the Pension Consulting
Agreement.

Clients may terminate the agreement without penalty for a full refund of CCAM’s fees within five (5) business
days of signing the Pension Consulting Agreement. Thereafter, clients may terminate the Pension Consulting
Agreement immediately upon written notice. CCAM bills based on the balance on the first day of the billing
period.

Fees are paid in advance on a quarterly basis. The advisory fee is calculated using the value of the assets in the
Account on the last business day of the prior billing period. Upon termination, for any unearned asset-based fees
paid in advance, the fee refunded will be equal to the balance of the fees collected in advance minus the daily
rate* times the number of days elapsed in the billing period up to and including the day of termination. (*The
daily rate is calculated by dividing the annual asset-based fee rate by 365.)

     B. FINANCIAL PLANNING SERVICES (PLANS AND CONSULTATIONS)

The Firm provides financial planning services that focus on needs and concerns specific to each client. These
services may be comprehensive in nature and focus on overall financial situation, risk, goals, and objectives;

these services may also be modular in nature, focusing on specific areas of concern that a client may have, such
as asset allocation, college planning, or estate planning. Financial planning services may include giving advice
on investment and non-investment related matters.

Firm representatives collect from clients a variety of information and documentation necessary to perform
requested services. Representatives gather information to review current financial conditions, to assist in
determining risk tolerance, and to identify financial goals, objectives, and challenges. Financial data may
include, but is not necessarily limited to, statements and account data from banks, broker-dealers, and mutual
funds, as well as tax returns and insurance policies. The Firm and representatives rely on the information
provided by clients. It is, therefore, critical that information provided to the Firm is complete and accurate.
Further, it is not the responsibility of the Firm or the Firm’s representatives to verify the accuracy or truthfulness
of information provided by clients. In addition, if authorized by a client, representatives will gather information
or documentation from other professionals and are expressly authorized to rely on provided information. It is
strongly recommended clients work closely with other professionals in their employ, such as attorneys or
accountants.

Firm representatives will discuss recommendations with the client, including steps to take in order to implement
said recommendations. It is the responsibility of the client to notify CCAM if there are any changes in the
client’s financial situation or investment objectives. A change in financial situation or investment objectives
may result in a reevaluation of current financial plans and may also result in a new or modified plan to meet
changed circumstances.

Although financial planning services are provided with the intention that the client will implement the
recommendations, the client is not obligated to do so. The client retains discretion over implementing decisions
relating to financial planning services and is free to accept or reject any recommendation from CCAM. The
client is free to purchase investment products that are recommended through other brokers or agents that are not
affiliated with the Firm.

Item 7 Types of Clients

Crystal Cove Asset Management provides investment advice to many different types of clients. These clients
generally include individuals, high net worth individuals, pension and profit-sharing plans, charitable
organizations, and corporations or other business entities.

The Firm does not require a minimum account size for any of its services.
